Files should be uploaded to per year/month directories by default in expression engine

Uploading thousands or millions of files into one directory is a very bad idea, which can have performance implications too. There are reports of sites going down just as a result of this issue.

How are the month (and year) folders automatically added to the uploads directory? I want to be able to add the new folder(s) each month as they are created. Is there a filter or action related to this process?
