LG Polls crashes site.

I’m running LG Polls on my site and it recently has just started crashing. When a poll is entered (with the associated answers) the site renders an error that “No poll found for entry: 5941”. This is remedied by GOING BACK into LG Polls and entering the answers that, for some reason, didn’t get attached. Any ideas?
