I’m looking forward to getting this extension/module working. I’m only interested in sending pingbacks when a blog entry has been published.
I uploaded the files (Pingback 1.21b) to their respective locations and then this was displayed at the top of the Control Panel when I went to the Extension Manager page:

I went to the Modules section and the message was still there. I then Installed the Module and the message at the top now said

At that point, I removed the module and uninstalled the extension.
I’m running EE 1.6.8 Build 20100121 and I also have MSM and the Discussion Forum module installed.
Hope this gives you some clues.

A warning because the module was written for PHP 4. Has been taken care of in version 1.2.2.
