If I want to use this for blog entries and comments, where in the template do I place the expression code?

You would find the templates that show your entries, and find the {body} and {extended} tag and surround it with the plugin tags. Then do the same for comments.

Is there any way to adapt this plug-in for use with the forum module? Specifically, I’d like to have the functionality of the plugin in forum posts. Can this be done?

In the forum preferences you have the option of choosing the “Text Formatting in Posts”, which is typically a plugin (or one of our three default options). To use Scripturizer would likely require making your own plugin that called both it and one of the other formatting options like XHTML or Auto <br>.
