EE Poll 1.7 not working on site (www.wneg32.com - front page)

We have successfully installed and used the EE Poll module on several of our sites. However, on the last site that we launched the ee poll does not work.

We have inserted the code to show the poll on the front page, and it displays correctly.

When you try to vote the vote does not record, and the results page is not shown.

Each poll is set to use cookies, and not IP addresses. Now, I have also tested the poll with the cookies and IP tracking turned off, and it still does not record the vote or show the results.

We have reinstalled the module as well, and that did not solve the problem.

Any help would be appreciated. Please let me know if any other info is necessary to help solve the issue.
